Not sure of the reason why this occurred but most likely a new identity has been created. Check the Users->username->Documents->Microsoft User Data->Office 2004 Identities folder to see if there are multiple identities folders in there. Typically the default identity used is called, "Main Identity". Switch back to that identity if for some reason a new one was created. To switch back, open Entourage and got to Entourage->Switch Identity, and then select Main Identity. This should bring it back. Also if you've rename ANY folders in the path to the Main Identity folder, Entourage would most likely recreate a new identity since its lost its connection.
